Most brain injuries occur without warning and the effects last a lifetime.  Families and individuals face many challenges including the initial hospitalization normally in critical care with multiple surgeries, inpatient rehabilitation, extended outpatient rehabilitation, and setting up a care plan for the rest of their life.  Midwest Care Management Services can help in each phase by:

 

  • Meeting personally with hospital staff to determine the patient's current condition, prognosis, and proposed medical procedures
  • Assisting in finding the best inpatient rehabilitation facility and coordinating transfer of patient to rehabilitation facility
  • Monitoring progress of rehabilitation and attending staffings
  • Communicating with medical professionals (physiatrist, physical therapists, occupational therapists, speech therapists) and providing information to family members
  • Applying for all public benefits including SSI, SSDI, and Medicaid.
  • Recommending and implementing care plans once an individual is ready for discharge from inpatient rehabilitation
