challenges including the initial hospitalization normally in critical care with multiple surgeries, inpatient rehabilitation, extended outpatient rehabilitation, and setting up a care plan for
the rest of their life. Midwest Care Management Services can help in each phase by:
- Meeting personally with hospital staff to determine the patient's current condition, prognosis, and proposed medical procedures
- Assisting in finding the best inpatient rehabilitation facility and coordinating transfer of patient to rehabilitation facility
- Monitoring progress of rehabilitation and attending staffings
- Communicating with medical professionals (physiatrist, physical therapists, occupational therapists, speech therapists) and providing information to family
members
- Applying for all public benefits including SSI, SSDI, and Medicaid.
- Recommending and implementing care plans once an individual is ready for discharge from inpatient rehabilitation
